Items sent prior to admissions or to be brought in person on day of admission:

  • Birth certificate
  • Social Security card
  • Immunization records
  • Documentation of dental exam (if completed within the past 12 months)
  • Last report card
  • Insurance cards
  • Any court documents applicable to custody rights of parents/guardians
  • All of current meds, we will review and return to you those we don’t need.
  • The most recent Discharge paperwork (if applicable), specifically regarding medications

Admission of your child will be unable to proceed without proof of immunizations, court documents and all guardian consents to admission.

Items to pack:

  • Seven shirts/blouses
    • no midriff, spaghetti straps, low dip plunging v-neck, off the shoulder, sleeveless (we are allowing sleeveless if we say they can go to edge of shoulder), tank tops
    • shirts are to go to the edge of the shoulder
    • no offensive designs/curse words, demonic, drug, gang, or sexual connotations/content
  • Seven pair of pants/shorts
    • no ripped, torn or pant/shorts that have holes from wear or damage
    • no pants/shorts with fashion tears above the knee, shorts must cover entire thigh.
    • Leggings must cover all skin to ankle; shirt length need to be long enough to cover torso or below when wearing leggings
  • Seven pair of underwear/bras
    • no underwire
    • no revealing or exposed undergarment
    • no body contouring or body hugging clothing
  • Seven ankle or footie pairs of socks
    • no calf length
  • One pair of each:
    • comfortable slip-on or Velcro shoes
    • athletic
    • slides or crocs
    • shower shoes
  • Seven pair of pajamas/shorts/tee shirts for sleeping
    • no ties or ribbons
  • One of each allowed:
    • coat
    • raincoat
  • Two of each allowed:
    • light jacket/hoodies without draw strings or ribbons
  • Personal hygiene products
    • shampoo/conditioner/body wash, mouth rinse/toothbrush/toothpaste, deodorant, brush/comb
    • non-alcohol based products only
  • Summer only, Pool schedule is May-September
  • Two of each allowed:
    • swim shoes,
    • one piece swimming suit
    • swim trunks along with swim t-shirts
    • no ties or strings

Items you MAY bring:

  • Hair dryer, cordless electric shavers
    • rechargeable is recommended
  • One each: chap stick/ lip balm
  • One each: Cards, puzzles, word searches
    • basic only, not trading or character cards
    • staples removed from word search books
    • No Metal in book/booklets, including staples
  • Sunglasses, hat – No Metal in frames, screws or wires
  • Up to 2, maximum of 12” tall, toys or stuffed animals
  • NOTE: toys or stuffed animals 12” or smaller can be taken off the unit.
  • One medium-size stuffed animal or toy, up to 20” tall can be kept in patient room.
    • No rope, zipper, strings, or holes in toy or stuffed animal.
    • No Legos
    • No battery operated or magnetic toys or “Contraband “items, see below.
  • Up to 2, hand-held stress balls or swish balls
  • Up to 4 age/content appropriate notebook-size books
  • 1 journal, 1 coloring book,  1 sketch pad
    • staples removed
    • no metal spirals and no wires
  • 1 pack non-toxic, broad-tip markers and/or crayons
    • No metal, sharpener or fine point
  • Up to $20.00 for haircuts or outings
    • will be kept in Business Office
  • Twin-sized personal pillow, blanket, bedspread
    • All of our beds are twin-sized
    • no bed sheets
  • Stamps, envelopes, stationary
  • Noise-cancelling headphones or ear plugs
    • No metal
    • Non-electric
  • Weighted blanket
  • Silicone spacers only for ear piercings
    • do not bring extra
  • Several family member pictures, including pets, without frames
    • No original pictures, nothing that cannot be replaced
    • Small photo album – without metal
    • no alcohol/drugs or inappropriate clothing in pictures

Items that are not allowed:

  • Jewelry of ANY kind
    • No necklaces
    • No metal items
    • Silicon spacers may be worn in place of earrings
    • Piercings – Body, facial, tongue studs, loops or gauges
  • Make-up
  • False nails, false eyelashes, or metal barrettes – please remove before admitting
  • Cell phones, radios,  mp3 players, cameras, flashlights, or any electronics entertainment devices
  • CDs, DVDs, videos or tapes, video games or any battery operated items, including watches
  • Musical instruments
  • Razors, knives, sharp edge objects or weapons,
  • Matches, lighters, vapors or any type of tobacco products or accessories
  • Glass items, i.e. – compacts or mirrors
  • Hard plastic or metal items, picture frames
  • Steel-toed or cowboy boots
  • Posters, clothing or books with drug, gang, sexual or demonic connotations/content
  • Snacks or foods unless on a special diet arranged with campus dietitian
  • Aerosols, nail polish/remover, cologne/perfume or any items containing alcohol,
  • Pens or permanent marking items or sharpener’s with blades
  • Belts, shoe laces, clothing with ties, paracords/survival bracelets, bandanas, scarves, or ligature items
  • Fidget Spinners
  • Flip Flops
  • Any alcohol based products
  • Anything valued over $50
  • Anything battery operated
  • No magazines
