Clinical professionals can refer patients to Meridell Achievement Center by filling out the contact form below. Please complete and submit these admissions forms when you make the referral.

For assistance, please call 800-366-8656 and ask for Intake Services. Hours are Monday to Friday from 8:30 a.m. to 5:00 p.m. If you call after hours, leave a message for the next weekday.
