Each patient is asked to complete a satisfaction survey when discharging from Meridell. Each question is rated on a scale from 1 to 5 (1 = Strongly Disagree, 2 = Disagree, 3 = No Opinion, 4 = Agree, 5 = Strongly Agree)
Each patient’s family is asked to complete a family satisfaction survey when patient discharges from Meridell. Each question is rated on a scale from 1 to 4 (1 = Poor, 2 = Fair, 3 = Good, 4 = Outstanding)
